PT Case Manager - Provides case management to our enhanced care services team in Hood River. Must possess knowledge of and challenges faced by elderly people with mental illness. Position is a .6 FTE (22.5 hrs/wk) with weekend and evening hours. Salary starts at $2072.40/mo. Closes 5/30/12.
